Crisis Recovery Fund

Complementing the work of Comprehensive Crisis Services (CCS), the Fund provides resources to aid local families affected by violence, suicide and other critical events. CCS provides acute behavioral health services, donations to the Fund assist grieving families with funeral costs, temporary housing, food and other immediate needs. Information at 415-970-3800

End Hep C SF

Is a multi-sector collaborative initiative that utilizes evidence-based practices, community wisdom, and the creative leveraging of resources to work toward hepatitis C elimination in San Francisco. Information at endhepcsf.org.

Getting To Zero SF

San Francisco’s Getting to Zero initiative is a multi-sector, independent consortium operating under the principles of collective impact. Modeled after the UNAIDS goals, GTZ-SF’s vision is to reduce HIV transmission and HIV-related deaths in San Francisco by 90% before 2025. Information at gettingtozerosf.org.

Gender Health SF

Provides access to transgender surgeries and related education and preparation services to eligible uninsured transgender adult residents. Additionally, provides a range of health services to transgender residents such as primary care, prevention, behavioral health, hormone therapy, specialty and inpatient care. Healthy San Francisco

Healthy San Francisco is a program designed to make health care services available and affordable to uninsured San Francisco residents. It is operated by the San Francisco Department of Public Health. Information at healthysanfrancisco.org or by calling 415-615-4555.

IRGT

The IRGT works with trans organizations, communities, and advocates around the world to aid in strategic planning, programming, service delivery, and high-level advocacy for transgender communities. Information at https://irgt.org/

Jail Health Services

Providing a comprehensive and integrated system of medical, psychiatric and substance abuse care to inmates of the San Francisco County Jail system. JHS staff triage over 31,000 prisoners annually. Information at 415-995-1700.

One Love Black Community

At the beginning of COVID19 a Black Midwife in San Francisco, Asmara Gebre, noted a racial disparity among recipients of pantry resources being offered in her clinical setting. This inequity inspired Asmara to create a resource that centered on serving the Black community, and on April 25th, 2020 One Love Black Community was born. One Love Black Community’s mission is to earn the trust of our community through action–we show up with essential resources, solidarity, love, respect, humility, and more. As a grassroots, Black-Led community endeavor, we show up for Black people accessing reproductive health care services in San Francisco. Our vision is rooted in building meaningful relationships in our Black community, while offering direct access to essential resources with no strings attached or questions asked (items include, but not limited to, food, toiletries, household essentials).

RPITF – Regional Pacific Islander Task Force

The Regional Pacific Islander Task Force works in San Francisco, San Mateo, and Alameda counties to ensure that the health disparities and health needs of the Pacific Islander communities in those counties are considered in public health decision-making.


Transitions Clinic Network

Launched in 2006, Transitions Clinic Network (TCN) works upstream to transform health systems by building capacity in primary care programs providing healthcare and social services to support successful re-integration for individuals returning from incarceration. TCN sites all train and employ community health workers with histories of incarceration as part of their care teams. Developed at a clinic site in San Francisco, the TCN model has been implemented by dozens of programs, evolving into a robust national network of clinics and advocates.
